The Eye Connect Healthcare Award is proudly developed by Macular Disease Foundation Australia (MDFA) to recognise outstanding eye healthcare professionals who have made a meaningful difference to the lives of patients and families.
Terms and Conditions
- 1. Entry is free and no purchase or donation is required to participate
- 2. Nominations are open to Australian residents aged 18 years or over.
- 3. Eligible nominees include qualified Australian optometrists, orthoptists and ophthalmologists currently practising in Australia.
- 4.MDFA Employees, Board members and Community Reference Group members of MDFA, and their immediate family members, are not eligible to be nominated.
- 5. Nominations must be submitted via the official MDFA nomination form during the published nomination period.
- 6. Each person may submit one nomination per healthcare professional.
- 7. Nominations must include:
- – the nominee’s name and profession;
- – the nominator’s contact details; and
- – a supporting statement of 200 words or less outlining why the nominee deserves recognition.
- 8. Nominations must be respectful, truthful and not contain offensive, defamatory, misleading or inappropriate content.
- 9. By submitting a nomination, nominators confirm that the information provided is accurate to the best of their knowledge and consent to MDFA contacting them regarding their submission.
- 10. Nominations will be reviewed by MDFA’s Consumer Reference Group against the published judging criteria.
- 11. The Consumer Reference Group will select up to five finalists and one overall winner.
- 12. Judging decisions are final and no correspondence will be entered into.
- 13. Finalists and the winner will receive a commemorative trophy only. No cash prize or financial benefit will be awarded.
- 14. MDFA may publish finalist and winner names, nomination excerpts and photographs for promotional and awareness purposes. Consent will be obtained from finalists and the winner prior to publication of personal stories or images.
- 15. Personal information collected through the nomination process will be handled in accordance with MDFA’s Privacy Policy and applicable Australian privacy laws.
- 16. MDFA reserves the right to verify nominee eligibility and to withdraw or amend the award process if required for fairness, safety or compliance reasons.
- 17. MDFA reserves the right to decline nominations that do not align with the values and purpose of the award.
Judging Criteria
Nominations will be assessed on:
- – Demonstrated compassion, empathy and patient-centred care;
- – Positive impact on a patient’s quality of life;
- – Commitment to communication, support and education;
- – Professionalism and excellence in eye healthcare; and
- – Contribution to improving the patient experience.
