Macular Disease Foundation Australia Professional Friends Program: Terms and Conditions
By signing the Macular Disease Foundation Australia (MDFA) Professional Friend Program (Program) Application you agree to the following Terms and Conditions.
Membership of the Program is not an endorsement or recommendation by MDFA of the member optometrist and should not be represented as such by any individual or practice.
1. Professional Friend Program
Program membership is conferred upon an individual optometrist (Program Member). If the Program Member operates more than one practice the benefits of the Program will extend to the other practices up to a limit of 3 practice locations. Program membership is not transferrable to another individual. Program benefits and cost are outlined in the invitation letter. These may be amended by MDFA from time to time. Membership of the Program is considered effective from the date payment is received, for one year. Membership may be renewed annually by payment of the annual membership fee in accordance with item 6.
2. Practice display materials
Program Members will receive a certificate of membership and a decal that can be displayed within each practice location under the Program membership. A Program Member is permitted to display the Program certificate and decal (or other materials as supplied by MDFA from time to time) in relation to their membership, for the duration for which their membership remains current. If Program membership is terminated (see item 8 for more detail), the Program Member must remove all Program materials from display and cease to represent themselves as a Program Member. Program Members must not use any trade marks owned by MDFA other than by displaying Program certificate and decal, and any other the materials supplied by MDFA in accordance with this item 2.
3. Web directory
MDFA will have a national database of optometrists on its website from early 2014. MDFA's web-based national directory of optometrist has been drawn from the listings on the OAA website. Program Members will have their listing highlighted as being a member of MDFA's Professional Friend Program. MDFA will use its best endeavours to ensure that member information displayed on the web-based national directory of optometrists remains current but accepts no responsibility for the accuracy of the information provided by the Program Member. Responsibility for the accuracy of the information provided to MDFA lies with the Program Member. Any changes required to a member’s information as displayed in the web-based directory must be requested in writing to MDFA. MDFA will amend the listing as soon as practicable.
4. Resource orders
Program Members can order MDFA publications and resources free of any postage and handling charges. Free postage and handling on resource orders applies to a maximum of one order per calendar month for each practice location. Orders must be placed on the MDFA Professional Friend Program Resource Order Form which will be provided and include the Program membership number. Orders placed by any other means will incur the standard postage and handling fee.
5. Use of information
Information provided to MDFA by the Program Member will not be used for commercial purposes but MDFA will publish your contact details in its online web-based directory. Except via its web-based directory, MDFA will not sell or intentionally make available to any other organisation information provided by the Program Member. Should MDFA receive a request from a Program Member to be removed from the web-based directory or to not receive communication from MDFA, MDFA will comply with this request at the earliest possible convenience.
6. Payment
The annual membership fee for the Program is $200 + GST. Fees may be paid via credit card or cheque. Payment via EFT may be arranged if required. For payment via credit card, the fee will automatically be charged to the credit card provided at the time of application at each annual renewal unless instructed otherwise by the Program Member or if the Program membership is terminated. For payment via other means, the Program Member will be contacted 1 month prior to the annual Program renewal date to arrange payment of the annual membership fee. Payment must be made within the time period specified in the notice.
7. Dispute Resolution
Both MDFA and the Program Member will respond to any queries, issues or complaints submitted in writing by either party within 5 working days. Each party will use their best endeavours to resolve any issues.
8. Termination of Program membership
Program membership may be terminated by the Program Member at any time by writing to MDFA. MDFA may terminate Program membership if the member fails to adhere to the Program Terms & Conditions and there has been no acceptable outcome to any dispute resolution (see item 7), or if Program membership fees are unpaid. MDFA will not provide any Program membership fee refund irrespective of the reasons for termination.
